I hereby release Fence Guys, my former employers, and all other persons from any and all claims, demands, or liabilities arising out of or in any way related to such inquiry or disclosure. I understand that Fence Guys is committed to maintaining a drug and alcohol-free workplace. Accordingly, I may be subject to a blood test, urinalysis, or other drug/alcohol screening. I further understand, I may be subject to such random or routine drug and alcohol screening if Fence Guys has reasonable suspicion to believe that I am under the influence of a drug or alcohol. My consent to submit to such a test is required as a condition of working with Fence Guys and my refusal to consent shall result in refusal to assign work or termination of work. I understand and agree that my offer of work assignment or assigned work is established with no guarantee or promise of continued work or set hours of work or any obligation on the part of Fence Guys beyond pay for actual work performed. I further understand that this work assignment is at-will and may be terminated at any time with or without cause, without prior notice, by either myself or Fence Guys. There will be no agreement, expressed or implied between Fence Guys and myself for any specific period of work assignment, nor for continuing employment, unless made in writing, signed by an authorized representative of Fence Guys.

I, the undersigned applicant, do hereby certify that all the information provided by me for the purpose of work assignment is true and complete to the best of my knowledge. I understand that falsification of any information on company documents may lead to refusal of work assignment or termination of work assignments. I understand that investigative background inquiries will be made about me that can include education verification, criminal convictions, and motor vehicle records. These reports will include information as to my character, general reputation, work habits, performance, and experience. Further, I understand that Fence Guys will be requesting information from various federal, state and other agencies that maintain records concerning my past activities relating to my driving, credit, criminal, civil and other experiences. I authorize, without reservation any party or agency contacted by Fence Guys and/or their appointed representatives to furnish the above-mentioned information prior to or at any time. The information on this form will be used solely for the purpose of conducting background checks to determine work assignment eligibility and will be maintained in a confidential file. I hereby release all persons and agencies providing such information from any and all claims, damages, or liabilities connected with their release of any information. I agree that any copy of this document is as valid as the original. I do hereby agree to forever release and discharge Fence Guys to the full extent permitted by law from any claims, damages, losses, liabilities, costs and expenses or any other complaint filed with any agency arising from retrieval and reporting of information.
